For the following accredited entities or individuals: 

• Corporations, partnerships, limited liability companies, tax-exempt organizations, trusts and other entities with total assets of more than $5 million.
• Individuals with an annual income of more than $200,000, or joint annual income with their spouse of more than $300,000, for each of the last two years. 
• Individuals with a net worth, or joint net worth with their spouse, of more than $1 million.

Please upload one of the following: 

 1) Most recent tax returns, IRS Form 990, or other IRS tax filing to support your accredited status

OR

2) A written letter from a registered broker-dealer, investment adviser, licensed attorney in good standing, or certified public accountant registered and in good standing, stating that they have taken reasonable steps to verify the entity’s/person’s accredited investor status within the past three months and have determined that the entity/person is an accredited investor.
